Thank you for taking the time to review the Open Arms Link Employee Handbook. To ensure that we are aligned on company policies and expectations, we kindly request your acknowledgment of the handbook's contents. Please follow the instructions below to complete the acknowledgment process:
Step 1: Print Acknowledgement Forms

Step 2: Sign the Acknowledgement Form
Read the content of the form, ensuring you understand its significance.
Print your full name legibly at the bottom of the Employee Handbook Acknowledgement Form.
Sign and date the form at the bottom where indicated.
Step 3: Initial Each Policy
For each policy or section you read, locate the space provided next to it.
Initial in the designated space to indicate that you have reviewed and understood that policy.
Print your full name legibly at the bottom of the form.
Sign and date the form at the bottom where indicated.
Step 4: Return the Completed Form
Once you have initialed each policy and signed the forms, submit the completed forms to your supervisor.
Your supervisor will review and submit forms to the Human Resources Department
